Newton and Haydock Primary Care Network have established a frailty team to deliver care to older people living with frailty - both in their own homes and in nursing homes in the area. 

This is an innovative service to improve the quality of care and treatment for frail people, that we hope will serve as a model of good practice both within the region and nationally. 

The team works across all of the Newton and Haydock GP practices, and liaises closely with community matrons and the St Helen and Knowsley Trust frailty team.
